The right question:

“What’s working, and how can we do more of it?”

The wrong question:

“What’s broken, and how do we fix it?”

Look for the bright spots, the places where things are better — study them, copy them, spread the practices.  This may be easier said than done, but it is absolutely critical to positive change.
